INSTRUCTIONS FOR COMPLETION AND SUBMISSION
All Participating Agencies must complete the Forensic Services Agreement and have it signed by the
agency’s Chief Administrator. (e.g., Chief of Police, Sheriff, etc.). This must occur before the Participating
Agency will be granted access to the Pre-Log website. Once the Division of Forensic Services implements the new
Laboratory Information Management System (LIMS), all Participating Agencies must utilize the Pre-Log website to
initiate and track the submission, analysis and retrieval of evidence.
All Participating Agencies must designate a Lead Administrator. The Lead Administrator will be provided access
to the Pre-Log website and have the ability to create and manage additional users in Pre-Log for only that
Participating Agency.
1. Participating Agencies must obtain a copy of the Forensic Services Agreement that is automatically generated
by this website. To generate the document, the Participating Agency must click on the "Enter Your User Agreement"
button a the bottom of this web page and enter the required information. Be sure to have your agency information
ready to complete the agreement.

2. Be sure to have your agency information at hand when you complete the agreement. You must submit the agreement (on the
next page) from a Physically Secure Location at your agency. A Physically Secure Location means a facility, police vehicle,
or an area, room, or group of rooms within a facility with both the physical and personnel security controls sufficient to
protect Criminal Justice Information (CJI) and associated information systems. [Federal Bureau of Investigation, Criminal Justice

3. After the Forensic Services Agreement has been downloaded, the documents must be signed
by the Chief Administrator (highest ranking official or a senior level executive with the authority to bind the agency).
Ink or electronic signatures are acceptable. An electronic copy of the signed agreement must then be emailed to
ISP.LIMS@illinois.gov from the Lead Administrator's official email address for that Participating Agency.
4. Upon successful completion of this process, the Division of Forensic Service will review the submitted
information grant the Participating Agency access to the Pre-Log website.
5. The individual identified as the Lead Administrator for the Participating Agency will receive an
email that will include a username and default password to access the Pre-Log website.
6. The Division of Forensic Services will provide Participating Agency with video training and
documentation on how to utilize the Pre-Log website once registration is quality reviewed and accepted.
